	Form D - Application for Employment [DOWNLOAD] The completed application must be submitted to the clerk's office. Applicant must live within 15 miles of the corporate limits of the City of Kendallville. Applicant must have NO criminal record. Application will be accepted for advertised positions only. 	Fire Department Volunteer Application [DOWNLOAD] Individuals must be at least 18 years old, must live within the Kendallville zoning limits, have NO criminal record, be in good physical condition, and be willing to complete the essential training to meet all federal, state and local fire department requirements Contact the Fire Department for more information.
	Request for Fire or Incident Report Application [DOWNLOAD] The request must be formally requested in writing to our office, not by a telephone call. The documents will be either mailed or can be picked up by the individual.  The requested items will not be faxed. Contact the Fire Department for more information.
	Form EE - City of Kendallville Request for Inspection/Release of Public Records [DOWNLOAD] The request must be formally requested in writing to our office, not by a telephone call. The documents must be paid before we can release them. The documents will be either mailed or can be picked up by the individual.  The requested items will not be faxed.
